You write the questions

There is no fixed template to work around. Name the form, write the questions in your own words, choose which of your services need it, and change it whenever you like. The wording is versioned, so a signature always points at what was actually on screen that day.

Kept with the customer

The signed copy sits on the customer record with the date, the answers and the exact wording they agreed to. When somebody rings up disputing what they were told, it takes seconds to find.

Deleted when it should be

You set how long records are kept and they are removed on time, without you remembering to do it. Keeping consent records forever is its own problem.

During the booking. The questions become part of the last step. Somebody booking a service that needs authorisation answers before the booking exists at all, so there is no such thing as an unauthorised job on your diary.

Before the appointment. For anything with a waiting period — a patch test that has to be done 48 hours ahead — the link goes out with the confirmation email. The booking is held, and you can see who has completed it and who has not without opening each one.

When they arrive. For a form that only makes sense in person, staff can open it on the day and have the customer sign on the screen.

What happens to a flagged answer

An answer you have marked as one that matters does not just get recorded. The booking is flagged, so the person doing the work sees it before they start rather than after. What you do about it is your judgement — the system's job is to make sure nobody finds out by accident.

What it is not

It is not legal advice, and it does not decide what you should be asking. Every trade has its own obligations and its own insurers, and the wording is yours. What this does is make sure the asking happens, gets recorded, and can be found again.

Questions people ask

No. You write your own forms — a name, your questions, a declaration, and which services need it. Nothing has to be approved by us first.

That is your choice, per form. Set it to be answered during the booking and the booking cannot be completed without it. Set it to go out by email and the booking is taken and held while you wait.

It is recorded with the date and time, the exact wording that was shown, and the answers given, and it cannot be edited afterwards. That is a far better record than a signature on a form nobody can find.

You stop being able to create or send new forms. Everything already signed stays readable — those records are evidence and are not ours to take away.
